Job Summary
We are seeking a dedicated and highly skilled Hospital Pharmacist to join our dynamic healthcare team. In this vital role, you will be responsible for ensuring the safe, effective, and efficient medication management within our hospital setting. Your expertise will directly impact patient outcomes by providing comprehensive pharmaceutical care across various departments, including emergency, ICU, pediatrics, and trauma units. Must have Hospital or in-patient experience.

Qualifications* Active SC Pharmacist License* 1 year of recent experience in an in-patient Hospital/Rehab/LTAC setting

Responsibilities

  • Dispense medications accurately and efficiently in accordance with physician prescriptions and hospital protocols, ensuring adherence to aseptic techniques and safety standards.
  • Collaborate with healthcare professionals to develop and implement medication therapy plans tailored to individual patient needs, including acute pain management and critical care scenarios.
  • Conduct thorough patient assessments, including evaluating physiology, anatomy, and medical terminology to determine appropriate medication regimens.
  • Manage IV infusions and sterile compounding processes with precision, maintaining strict aseptic techniques to prevent contamination.
  • Provide expert guidance on pharmacy-related issues such as managed care strategies, medication administration protocols, and hospital medicine practices.
  • Monitor patient responses to medications, adjusting therapy as needed while ensuring optimal outcomes in high-pressure environments like Level I Trauma Centers or PICU settings.
  • Maintain accurate documentation of medication dispensing, patient interactions, and clinical interventions while complying with regulatory standards.
  • Stay current with advancements in pharmacology, including emergency room protocols, ICU practices, and post-residency clinical pharmacy experience requirements.

Experience

  • Proven hospital pharmacy experience with a strong background in critical care environments such as ICU or Level I Trauma Centers.
  • Demonstrated expertise in managed care principles and medication management within complex healthcare settings.
  • Extensive clinical pharmacy experience focusing on patient service, patient care, and medication safety across diverse departments including pediatrics and emergency rooms.
  • Prior experience working in high-acuity areas like PICU or trauma units with an emphasis on acute pain management and emergency response procedures.
  • Completion of a pharmacy residency program or equivalent post-residency experience emphasizing hospital medicine and clinical pharmacy practices.
  • Familiarity with physiology, anatomy knowledge, medical terminology, aseptic techniques, IV infusion preparation, and medication administration protocols.
  • Strong understanding of hospital operations combined with excellent communication skills to collaborate effectively with multidisciplinary teams for optimal patient outcomes.
